Our list is open and we are currently taking on NHS patients from within our catchment area. Our boundary lines for our catchment area are, roughly, North-West of our address (from Baytree Road towards the sea front).  If you are not sure if your address falls within our catchment area, please call the surgery on 01934 628118 and one of our Receptionists can advise you.

You can download and print a copy of both the registration forms below; please be advised you will need to attend the surgery in person with the correct identification to be accepted.

Identification Requirements

To register you will need to bring in to the surgery two forms of identification. One must have your current address on it, such as a Utility Bill or Tenancy Agreement. The second must have your photo on it, such as your Passport or Driving Licence. Without these documents we are unable to register you.
